Mental health staff indicated that follow-up contacts and relevant documentation are recorded in both the confidential Hampton-Newport News Community Services Board system and the facility’s HYPE electronic log. The auditor reviewed available intake assessments and related documentation, which supported compliance with required screening and follow-up procedures. Similarly, NNJDC Policy 115.381(B) requires that residents identified as having previously perpetrated sexual abuse be offered a follow-up meeting with a mental health practitioner within 14 days of the screening. Staff interviews confirmed this practice.   Policy 115.381(C) limits the dissemination of information related to prior sexual victimization or abusiveness to medical and mental health practitioners and other staff who require the information to inform treatment, housing, programming, and security decisions, or as otherwise required by law. The PREA Coordinator confirmed restricted access to sensitive records, and the auditor observed that resident files are securely stored in the intake area.   NNJDC Policy 115.381(D) requires that medical and mental health practitioners obtain informed consent from residents before reporting prior sexual victimization that occurred outside an institutional setting, unless the resident is under 18. Medical and mental health staff confirmed that they review consent with residents and their mandated reporting obligations at the outset of services.   Compliance Determination: Based on the evidence reviewed, the auditor determined that the facility meets the requirements of this standard and no corrective action is required. 115.382 Access to emergency medical and mental health services   Auditor Overall Determination: Meets Standard Auditor Discussion The following evidence was reviewed to determine compliance with Standard 115.382:   Documents Reviewed: ·                     NNJDC Pre-Audit Questionnaire
