Documentation Sampling
Where there is a collection of records to review-such as staff, contractor, and volunteer training
records; background check records; supervisory rounds logs; risk screening and intake
processing records; inmate education records; medical files; and investigative files-auditors must
self-select for review a representative sample of each type of record.
77. In addition to the proof
documentation selected by the agency
or facility and provided to you, did you
also conduct an auditor-selected
sampling of documentation?
Yes
No
78. Provide any additional comments
regarding selecting additional
documentation (e.g., any documentation
you oversampled, barriers to selecting
additional documentation, etc.).
All files selected for review were identified
using the same random selection
methodology employed for the interviews.
The auditor used a randomly generated list of
numbers corresponding to the rosters
provided by the facility to identify the files for
review.
The file review consisted of ten (10)
comprehensive resident files and ten (10)
employee training files/records, for a total of
twenty (20) files reviewed. Employee files
included documentation of background
checks, child abuse registry checks,
employment references, and records detailing
required training, including PREA training
documentation.
All fourteen (14) incident files were also
reviewed in totality by the auditor.
