The facility site review was conducted on the
first day of the onsite audit immediately
following the entrance meeting. A senior
administrative staff accompanied the auditor
throughout the review, providing guidance
and information regarding the facility’s
operations and physical layout.
During the site review, the auditor assessed
the facility’s physical plant, engaged with
staff, documented observations, and asked
questions regarding facility operations and
supervision practices. The auditor reviewed
the camera surveillance system and identified
camera locations throughout the facility.
PREA-related signage was observed in
multiple areas, including zero-tolerance
notifications and information regarding how
residents could contact the local advocacy
organization, the Center for Sexual Assault

on housing pods near resident telephones and
displayed on the lobby information screen.

in relation to §115.313. Areas observed during
the site review included the intake area,
outdoor recreation area, clinic, visitation area,
education hallway, classrooms, dining hall,
housing units, and storage areas. The auditor
also engaged in discussions with line staff,
medical staff, mental health staff, and
detention specialist supervisors.
There were no admissions on the day of the
onsite review; therefore, the auditor was
unable to observe an intake, screening, and
