and through ongoing education, including handbooks, brochures, and prominently

Residents may report concerns directly to staff, supervisors, or administration,
submit written grievances, or use the facility’s reporting phone line (#333) to
contact the Center for Sexual Assault Survivors (CSAS). Additional reporting
avenues include contacting family members, legal representatives, probation
officers, or other third parties who can report on their behalf. The facility also
maintains a third-party reporting form and provides reporting information on its
public website.
During interviews, 16 randomly selected residents were able to identify at least one
method for reporting sexual abuse or harassment, including use of the hotline,
speaking with trusted staff, or submitting a grievance. All residents also identified at
least one external reporting option such as contacting a family member or attorney.
Roughly half of the residents indicated uncertainty about anonymous reporting;
however, overall resident awareness of reporting mechanisms was high. Those that
were uncertain indicated that sharing their name would be beneficial for
investigation purposes but they were unaware as to the actual anonymity
expectations for hotline reporting.
Staff interviews confirmed that employees are trained to accept all reports of sexual
abuse or harassment seriously and to report them immediately without requiring
chain-of-command approval. Staff demonstrated knowledge of multiple reporting
avenues, including direct reporting to administration, use of the hotline, contacting
law enforcement (NNPD), or reporting to CPS. Staff also confirmed that all reports
are documented promptly in accordance with facility policy.
During the facility site review, the auditor observed reporting posters and
informational materials posted throughout housing units, intake areas, and common
spaces. A test of the resident phone system confirmed that the reporting hotline
was functional and accessible.
The PREA Coordinator confirmed that all reporting options are reviewed during
annual staff training and reinforced through refresher training and facility postings.
The facility’s grievance system also provides an additional written reporting avenue,
and staff confirmed that grievances involving sexual abuse or harassment are
handled outside of the normal grievance review chain to ensure impartiality and
confidentiality.
