Medical and mental health staff confirmed that residents placed in isolation would
receive daily check-ins to monitor physical and psychological well-being, and that
such visits would be documented in the resident’s file. Supervisory staff likewise
confirmed that any isolation placement would be reviewed promptly by
administration to determine whether alternative housing arrangements are
available.
According to the Pre-Audit Questionnaire, NNJDC did not place any residents in
isolation for reasons related to risk of sexual victimization during the 12-month audit
period. As a result, no related case files were available for review.
Policy further requires that residents placed in isolation under safety provisions
receive periodic reviews at least every 30 days to determine the continued need for
separation. Staff confirmed that while no residents were placed in isolation during
the audit period, procedures are in place to ensure such reviews would be
conducted and documented if applicable.
Compliance Determination
Based on a review of facility policies, resident records, staff interviews, resident
interviews, and observations made during the facility site review, the auditor
determined that NNJDC appropriately uses PREA screening information to inform
housing and program assignments, limits the use of isolation to last-resort
circumstances, and ensures continued access to services for any isolated residents.
Accordingly, the auditor finds that NNJDC meets the requirements of Standard
115.342, and no corrective action is required.
