NNRHA voucher program: inspections, rents and audit follow-up

Compare program certifications with inspection, rent-review and audit-correction records.

Overview

Review SEMAP certification, quality-control samples, Housing Choice Voucher findings and corrective actions. Match each certification and audit to its own reporting period.

Source notes

Housing certifications and audit samples cover different measures and periods; these records are not a before-and-after series. Separate audit samples do not establish conditions in every assisted home. Questioned costs are not a finding of fraud or a final repayment decision. Board authorization and a management correction plan do not establish HUD confirmation or independently verified closure. Later program reports provide context, and a financial-controls solicitation is not a completed review; neither by itself closes the voucher findings.

Timeline

Dates follow the cited records; document dates are labeled separately.

  1. February 28, 2025

HUD issues an earlier-period performance rating

HUD’s letter assigns a 79% Standard Performer rating for the fiscal year ended June 30, 2023 and calls for written reporting on correction of deficient indicators. The letter is included in the April 2025 board package. This is an earlier-period HUD determination, separate from the later audit and 2025 self-assessment. The table gives indicator 10 five points, while the narrative lists it among zero-rated indicators; clarification is needed.

  2. September 29, 2025

    Independent audit reports voucher-control findings

CliftonLarsonAllen issued a qualified Housing Voucher Cluster compliance opinion for July 1, 2023–December 31, 2024. Reported samples included six of six untimely quality-control re-inspections, 36 of 60 units without inspections on a biennial basis, and 44 of 120 files without a completed rent-reasonableness determination form. These are distinct tests, not a combined defect rate. Findings 2024-002 and 2024-003 list $5,915 and $45,998 in questioned costs, respectively; final disposition requires separate evidence.

  3. December 31, 2025 target

Corrective-action plan sets a completion target

NNRHA’s plan assigns the Director of Housing responsibility for responding to six findings and sets December 31, 2025 as planned completion. It commits to inspection-documentation and process reviews. The plan alone does not establish completion or independent acceptance.

  4. January 7, 2026

    Staff propose the 2025 self-assessment

The memorandum describes a January 1–December 31, 2025 self-assessment of 125 out of 145 points, or 86%, subject to HUD confirmatory review. The accompanying table assigns zero of 20 points for lease-up. The certification copy has blank signature and date lines. These materials support the proposed submission, without establishing HUD’s final rating or closure of earlier audit findings.

  5. January 20, 2026

Board authorizes submission of the certification

January meeting minutes in the February package record a unanimous vote approving the submission resolution. February meeting minutes in the March package record approval of the January minutes. This establishes the recorded Board authorization; the signed submission, receipt and subsequent HUD determination remain separate records to obtain.

Open questions

Records still needed (4)
  • Certification and sample records

Signed calendar-year 2025 certification, dated submission receipt, final HUD score notice and indicator results; de-identified supporting samples and inspection and rent-review logs tied to their reporting periods.

Likely custodian: NNRHA; HUD; independent auditor

The self-assessment and Board authorization do not establish HUD’s final determination. Compare each certification with the population and period it measures.

  • Corrective action and questioned costs

Finding-by-finding testing after the planned completion date, de-identified sample definitions and results, and auditor or HUD acceptance for findings 2024-001 through 2024-006. Exclude tenant names, unit addresses, household identifiers and individual financial information.

Likely custodian: NNRHA; HUD; independent auditor

Establish what was tested and whether each finding was resolved; a planned correction is not evidence of completion.

  • Questioned-cost disposition

HUD determinations, repayment or adjustment records and de-identified reconciliations for findings 2024-002 and 2024-003.

    Likely custodian: NNRHA Finance; HUD

Questioned amounts do not establish final repayment liability or loss.

Written response to the February 28, 2025 HUD notice, any required corrective-action plan, HUD acceptance and clarification of indicator 10’s table and narrative scores for the year ended June 30, 2023.
