Adult jail medical reporting after the in-house transition

The Sheriff's 2024 annual report resumed monthly medical reporting after the 2023 report described unavailable utilization statistics. Some 2024 summary fields do not match ordinary arithmetic readings of the displayed components. Definitions, source tables and correction history could resolve those questions.

Overview

Review aggregate public reporting and administrative handover at the adult jail. This is not a patient-care assessment. The current adult model uses the Sheriff's in-house medical team, not the historical Wellpath contract. No patient harm, missing medical records, intentional misstatement or contractual breach is established.

Source notes

Counts may represent overlapping service events rather than distinct patients. The source workbook and clinical definitions were not available, so numerical patterns do not establish their cause or underlying data accuracy. The 2023 gap and 2024 table questions are not shown to have the same cause. The linked 2025 report could not be retrieved during this review; that is an access limitation, not evidence that no report or correction exists. Existing aggregate administrative records can address this inquiry without patient information.

Timeline

Dates follow the cited records; document dates are labeled separately.

  1. Historical adult contract

Historical adult contract reporting and transition terms

The Sheriff–Wellpath agreement included periodic reporting, record retention and audit access, with incorporated provisions for records custody and transition cooperation. It also allowed termination without cause on ninety days' written notice. These terms do not establish what was delivered or whether either party breached the agreement. The City acted as procurement agent, not a party to this adult agreement.

  2. 2023 reporting year

    Unavailable statistics and reconstruction described

The Sheriff's annual report said utilization statistics were unavailable after Wellpath's departure and that staff were reconstructing information from daily logs. It described employee medical staffing for 2024 but did not establish a reconstruction completion date.

  3. 2024 reporting year

Monthly reporting resumes with internally matching category totals

The next report contained twelve populated on-site month columns. Each of the eight category totals agrees with its displayed monthly entries, affirmative evidence that reporting resumed.

  4. FY2025 adopted budget

Budget comparison is not audited realized savings

The City described funding for the in-house division and an estimated comparison with an outside proposal. Those were budget figures, not audited realized savings.

  5. FY2027 adopted budget

Division budget and authorized, not filled, positions

The Medical Division is funded at $4,607,103 with 28 authorized positions. Authorized positions do not establish filled posts or staffing on any particular shift.

  6. October 18, 2026 review

    What needs explanation

January's eight on-site categories sum to 500, while its monthly total displays 42. The displayed annual total, 6,079, is exactly the first six categories and the six shown in the chart; all eight sum to 6,088. A deliberate category exclusion remains possible. Sick Calls shows 2,603 annually and AVG 400, rather than the ordinary twelve-month mean of about 216.92, but AVG is not defined. The inpatient annual Average Length field is 90, matching 20 admissions plus 70 days. Those published totals yield a simple ratio of 3.5 days per admission, not a validated replacement clinical measure.

  7. October 18, 2026 source review

    Current adult model is in-house

The current adult model uses the Sheriff's in-house medical team, not the historical Wellpath contract. This is not a patient-care assessment.
